Alzheimer’s Disease and Related Dementias (ADRD) present complex challenges that impact both patients and caregivers across the continuum of care. This session will explore an integrated medical and wellness-based approach to addressing the needs of individuals living with ADRD and those who care for them. Participants will gain insight into evidence-based medical treatments, holistic wellness strategies, and community-based supports that promote quality of life and reduce caregiver burden. The session will also highlight interdisciplinary collaboration and culturally responsive practices that enhance person-centered care. Ideal for healthcare providers, social service professionals, and caregivers, this session offers practical tools and resources to support the well-being of both patients and their support networks.
This session will highlight the importance of interprofessional collaboration in promoting quality care, improving health outcomes, and enhancing the well-being of aging populations. Experts from diverse disciplines will share strategies for integrating the 4Ms into practice to create age-friendly, patient-centered care.
